Registration Forms and Procedures

for 2012/2013

Thank you for your interest in Lubavitch Educational Center. Below you will find the necessary information and forms

to apply to our schools:

• Lubavitch Preschool
• Lubavitch Elementary School
• Beis Chana Middle and High School for Girls
• Klurman Mesivta for Boys

PLEASE NOTE: Only one Registration Form needs to be filled out per family.

To Apply to
LUBAVITCH PRESCHOOL 

1. CURRENT AND NEW STUDENTS:  Please download and print out the Registration Guidelines/Application Form and Credit Card Billing Authorization Form below. If you are applying for a scholarship, download and print out the Application for Financial Assistance.

2. Fill out the forms and mail them with the required fees and documents to the Office of the Registrar, at the address indicated above.
3. Once your application has been accepted, you will receive a contract for the coming school year.
4. FOR NEW STUDENTS ONLY: Download and print the following three documents and submit them as soon as possible to the administrative office of the school your child last attended.  No applications for new students will be considered for admission without this documentation.

LUBAVITCH ELEMENTARY SCHOOL

1. CURRENT AND NEW STUDENTS:  Please download and print out the Registration Guidelines/Application Form and Credit Card Billing Authorization Form below. If you are applying for a scholarship, download and print out the Application for Financial Assistance.

2. Fill out the forms and mail them with the required fees and documents to the Office of the Registrar, at the address indicated above.
3. Once your application has been accepted, you will receive a contract for the coming school year.
4. NEW STUDENTS ONLY: If you are registering for the first time at Lubavitch Elementary School, download, print out and complete the following document and submit it to the Lubavitch Elementary School office.  No applications for new students will be considered for admission without this completed form.

5. Download and print the following three documents and submit them as soon as possible to the administrative office of the school your child last attended.  No applications for new students will be considered for admission without this documentation.
   
   Transcript/Records Release Form
   Teacher Student Evaluation Form (Download 2 copies, one for the Hebrew teacher and one for the General Studies teacher to complete.)

   Once your your child has been accepted by the school Principal for the coming academic year, and all required documents and fees have been submitted, you will receive a contract for the coming school year

BEIS CHANA MIDDLE AND HIGH SCHOOL FOR GIRLS

1. CURRENT AND NEW STUDENTS:  Please download and print out the Registration Guidelines/Application Form and Credit Card Billing Authorization Form below. If you are applying for a scholarship, download and print out the Application for Financial Assistance.

3. Once your child has been accepted by the school Principal for the coming academic year, and all required documents and fees have been submitted, you will receive a contract for the coming school year.

2. NEW STUDENTS ONLY: If you are registering for the first time at Klurman Mesivta (and your son did not attend Lubavitch Elementary School in the 6th grade), download and print out the following document.  Complete the form and fax it with the required documents to the number indicated on the form.

3. Once your son has been accepted to Klurman Mesivta for the coming academic year, and all required documents and fees have been submitted, you will receive a contract for the coming school year.
